Facilities Use Request Forms

If you would like to use any of First Presbyterian Church's facilities, please read the Facility Use Policy and complete the forms below that apply to you (as a member or nonmember) or your group to include the application, room set-up, and waiver and release. 

After you have completed the forms, please submit them to first@fpcpca.net or via fax at 601-268-0778. A committee will review the forms and get in touch with you regarding approval.
